This policy covers the website only. Protected health information created or received in the course of clinical care is governed by the practice's Notice of Privacy Practices and by applicable law, including HIPAA. The website's contact form is not a HIPAA-compliant channel and should not be used to transmit health information.

Information we collect

• Information you submit through the consultation request form: your name, email address, telephone number, optional biological sex and age range, the reason for contact you select, and any message you choose to write.
• Basic technical information your browser sends, such as IP address, browser type and the pages you visit, which may be recorded in standard server logs.
• The website does not use advertising cookies or third-party tracking pixels unless that changes and this policy is updated.

How we use it

• To respond to your inquiry and, if you wish, to arrange a consultation.
• To keep an internal record of inquiries so that none is lost.
• To maintain and secure the website.

We do not sell your information. We do not share it with third parties for their own marketing.

How it is stored

Form submissions are stored on the website's hosting server in an access-restricted file that is not reachable from the public web, and are also sent by email to the practice. Access is limited to the practice and its website administrator.

Retention

Inquiry records are kept only as long as needed to respond and to maintain ordinary business records, and are then deleted. You may ask us to delete your inquiry at any time.
